In primary dysmenorrhoea there is no underlying disease process. In secondary dysmenorrhoea the pain is due to some other underlying cause.
During the menstrual cycle, the uterus prepares for pregnancy by increasing the thickness of the endometrium. If pregnancy does not occur, this endometrial layer is shed off at the end of the cycle which causes the menstrual flow. The process of shedding of this layer involves constriction of blood vessels as well as contraction of the uterus to push out the tissue. It is this process that causes primary Dysmenorrhoea. The severity of the pain varies in different people. Various factors influence it, including age (severity decreases with increasing age), childbirth (severity decreases after childbirth), exercise status (severity decreases inactive people) etc.
Secondary dysmenorrhoea has many causes including endometriosis, uterine fibroid, adenomyosis, ovarian cysts etc.
